Preventative Health Worksheet

There is no cookie cutter recipe for preventive screenings and immunizations. Many organizations have set up guidelines to help provide patients and doctors guidance as to which preventative health care measures are necessary. Guidelines do not incorporate individual variation based on co-existing diseases, family history and medications that you are on. Discussion with your primary health care provider as to which tests/immunizations are needed assures testing is individualized.

The appendix includes worksheets to track the necessary tests, exams, screens, immunizations and medications that each individual should consider. These are general guidelines and should not be taken as gospel for every individual. Individuals have unique medical histories and each person should have his or her preventative health care individualized.

This worksheet is a general form that can be used as a guide to understand the recommendations for getting adequate preventative health care. This form gives the individual an opportunity to record the dates of the preventative testing done to assure that they are following the guidelines. This tool allows you to see what you are missing and discuss this with your doctor.

As you can see there is a lot of testing that needs to get accomplished. It is extremely easy for the physician to miss some of the recommended tests in his rushed 10-minute office visit. This is why it is critical that each individual take responsibility for his or her preventative health care.

All individuals should:

• Exercise
• Eat well
• Maintain a healthy weight
• Do not use tobacco
• Use alcohol in moderation if at all
• Do not use illegal drugs
• Talk to your doctor about healthy lifestyles
• Get your annual exam
• Get all of your preventative testing
• Get all of your immunizations
• Discuss taking medications to prevent disease with your doctor
